Dùng khi đơn vị muốn xác nhận năng lực thực tế trước, chờ nhân viên hoàn thành bài đánh giá kiến thức sau.
Use this when your organization wants to confirm real-world competency first, before the employee completes the knowledge assessment.

Lưu ý: Hồ sơ này sẽ ở trạng thái chờ cho tới khi nhân viên đăng nhập hệ thống HOSTRAIN/ATM Academy, thi đạt đúng nghề/bậc anh chị chọn bên dưới, và bấm "Gửi yêu cầu xác nhận" trên trang kết quả — hệ thống sẽ TỰ ĐỘNG khớp nối với hồ sơ này (theo email hoặc số điện thoại nhân viên anh chị nhập bên dưới, phải trùng với thông tin nhân viên dùng để đăng ký tài khoản) và hoàn tất ngay, không cần đơn vị làm gì thêm.
This record stays pending until the employee logs in to HOSTRAIN/ATM Academy, passes the exact profession/level selected below, and clicks "Send confirmation request" on their result page — the system will AUTOMATICALLY match this record (by the employee email or phone entered below, which must match what the employee used to register their account) and complete it immediately, no further action needed from your organization.
Lưu ý: trường hợp xác nhận không đúng sự thật hoặc không hợp lệ, người được xác nhận sẽ không đạt yêu cầu để được học, thi hoặc cấp chứng nhận và không được hoàn tiền.
Note: if the confirmation is untrue or invalid, the confirmed person will not meet the requirements to study, take the exam or be issued a certificate, and no fees will be refunded.

Email hoặc số điện thoại phải trùng với tài khoản HOSTRAIN/ATM Academy của nhân viên để hệ thống khớp nối tự động.
Email or phone must match the employee's HOSTRAIN/ATM Academy account for automatic matching.

Cần điền ít nhất 1 trong 2 (email hoặc điện thoại). At least one of email or phone is required.

Với tinh thần trách nhiệm và uy tín cao nhất của người đại diện đơn vị tại mục I, tôi xác nhận người có tên và thông tin tại mục II ở trên có đủ năng lực thực hiện kỹ năng nghề đối với vai trò và trách nhiệm được đảm trách trong suốt thời gian làm việc tại đơn vị đến nay.

With the highest sense of responsibility and integrity, I, the representative named in Section I, confirm that the person named in Section II has demonstrated sufficient occupational skill competency for their role and responsibilities throughout their employment at our unit to date.

Mục "Điểm kiến thức nghề trên hệ thống" sẽ tự động điền khi nhân viên thi đạt — không cần đơn vị làm gì ở bước này.
The "system knowledge score" item will auto-fill once the employee passes — nothing further needed from your organization at this step.

IV. Confirmation and signature of the authorized person

Giám đốc khách sạn/doanh nghiệp hoặc người đại diện có đủ thẩm quyền xác nhận toàn bộ nội dung đã khai ở các mục I, II, III là đúng sự thật: tải lên ảnh có chữ ký và con dấu của đơn vị, khai họ và tên và chức danh của người ký. Sau khi bấm gửi, hệ thống xuất file PDF của phiếu và gửi vào email của HosTrain, nhân viên và đơn vị để lưu giữ.

The Director of the hotel/company, or a duly authorized representative, confirms that all content declared in Sections I, II and III is true: upload an image showing the signature and the stamp of the organization, and enter the signer's full name and title. After you submit, the system generates a PDF of the form and emails it to HosTrain, the employee and your organization for the records.

JPG or PNG, max 5 MB: a photo or scan of the signed and stamped confirmation, or an image of the signer's signature + stamp. Clear, well-lit, with the signature and stamp clearly visible.
